Church Benevolence Request & Decision Log
For gifts to a benevolence fund to be treated as gifts to the church, the church must have full control and discretion over their use — a donor may suggest, but the suggestion has to be advisory. So this log is arranged around the decision: request in, considered by a body of the church, decision recorded with a date and the people who made it. It records declines as well as approvals, because a log showing only payments evidences money leaving rather than discretion exercised.
