If an organization used VolunteerBadge to run a background check on you — for example, to volunteer or help with a nonprofit, church, school, camp, or youth program — this page is for you. ScreenForge Labs LLC, operator of VolunteerBadge, is the consumer reporting agency (CRA) that prepared your report, and under the federal Fair Credit Reporting Act (FCRA, 15 U.S.C. § 1681) you have important rights. This page explains them and how to use them.
Your rights under the FCRA
- Know what's in your report. You can request a copy of the consumer report we prepared about you, after we verify your identity.
- Get a free copy in certain cases. Your copy is free if an organization took an adverse decision against you based on the report (request it within 60 days of that notice), and in the other circumstances the FCRA provides.
- Dispute anything inaccurate or incomplete. You can dispute information you believe is wrong or incomplete. We are required to conduct a reasonable reinvestigation — generally within 30 days — and correct or delete information that is inaccurate, incomplete, or unverifiable, at no cost to you.
- Be told when your report is used against you. If an organization decides not to move forward based on your report, it must tell you and identify us as the CRA. The decision is made by the organization, not by VolunteerBadge.
- Outdated negative information is limited. The FCRA restricts how long certain negative information may be reported.
- Your consent is required, and access is limited. Your report may only be obtained for a permissible purpose and with your authorization.

How to dispute your report
If you believe your report contains inaccurate or incomplete information, you can file a dispute — free of charge — and we will reinvestigate:
- Use our secure dispute form at screenforgelabs.com/disputes (select “VolunteerBadge”), or
- Email disputes@screenforgelabs.com.
Please describe the information you are disputing and include any supporting documents. Having your report or reference number handy helps us locate your file faster. We will investigate and notify you of the results, generally within 30 days.
How to get a copy of your report
To request a copy of the report we prepared about you, email disputes@screenforgelabs.com from the address associated with your screening, or write to us at the address below. We will verify your identity before releasing any report, to protect your information. If an organization took an adverse decision based on your report, request your copy within 60 days and it is free.
