Wildlife Rehabilitation Intake & Disposition Log

Wildlife rehabilitation permits commonly require a log of every animal taken into custody, and the fields recur across jurisdictions: date received, place of origin, species, condition on arrival, who brought it in, treatment, and disposition with its date. This workbook uses that shape and the short disposition vocabulary used in federal migratory bird reporting, computes the annual totals a year-end report usually asks for, and flags rows with a required field missing. It is NOT an official form, and where your permit conditions differ, they govern.
