Concussion & Return-to-Play Log

Every U.S. state has a youth-sports concussion law, and they generally require immediate removal on suspicion, written clearance from a qualified provider, and a graduated symptom-limited return-to-play progression. This log records all three — and its status column is written so it can never be misread as a clearance: it will not report a return until the written clearance is on file, a provider is named, every progression step has a date, and your state’s minimum interval has elapsed, and even then it says who cleared the athlete and when rather than drawing its own conclusion.

  • Graduated progression tracked step by step
  • Case status that names the provider rather than clearing anyone

Some states set a minimum. Leave blank only if you have confirmed there is none.
