Your SMART Health Card (for volunteers)

What a SMART Health Card is, how to get one free, and how to add it to your VolunteerBadge profile.

A SMART Health Card is a free, digital version of your immunization record — a QR code issued and digitally signed by a trusted source like your state health department, pharmacy, or health system. Because it's signed, anyone you share it with can confirm it's authentic without calling anyone. Over 150 million Americans already have access to one. If an organization you're volunteering with asks for it, you can add your SMART Health Card to your VolunteerBadge profile so a verified immunization record travels with your identity and background check — in one place, ready when you need it.

It's optional and you're in control

Sharing a SMART Health Card is completely optional and you decide whether to add it. It is a value-added benefit — not part of your background check and not a consumer report. VolunteerBadge confirms only that the card is authentic; it doesn't judge your health. You can ask us to delete it anytime at privacy@screenforgelabs.com. See our Privacy Policy (§7A).

How to get your SMART Health Card (free)

Most people can get one in a couple of minutes from any of these:

  • Your pharmacy (CVS, Walgreens, and many others) — look under your vaccine records for "download your digital vaccine card" or a QR code.
  • Your state health department — search "[your state] SMART Health Card" or "[your state] digital vaccine record."
  • A records app like MyIR Mobile or Docket in participating states.

You'll end up with a QR code (save the image or a screenshot) or a code that starts with shc:/.

How to add it to VolunteerBadge

  1. When you fill out a volunteer application that requests it, you'll reach an optional Immunization Record step.
  2. Upload your SMART Health Card QR image, or paste the shc:/ code.
  3. We verify it with the issuer and add the verified record to your volunteer profile. Prefer not to? Just skip the step and continue.

Carry it with you across organizations

Add your SMART Health Card once and it travels with you. Sign in at volunteerbadge.com/badge (a quick emailed code — no password needed), open Your immunization record, and add your card. It stays on your VolunteerBadge account so you can share it with any organization that asks — add once, reuse everywhere. You can remove it anytime.
